Medicaid/other federal or state government insurance: These are government insurance programs for persons of all ages whose income and resources are insufficient to pay for health care. Unfortunately, these patients are not currently eligible to participate in this program.
Medicare Part D: This is the drug prescription program associated with Medicare. Unfortunately, these patients are not currently eligible to participate in this program.
Private/commercial: This type of insurance includes individually owned, employee provided HMOs (Health Maintenance Organizations), EPOs (Exclusive Provider Organizations), PPOs (Preferred Provider Organizations), POSs (Point-of-Service plans), or HDHPs (High Deductible Health Plans).
No insurance: This applies if you are currently uninsured and not receiving Medicaid or other federal or state government insurance. The LEVOXYL $5-off Savings Card works for people who do not have insurance.
